Hospital executives are under enormous pressure to “do something with AI.” Vendors promise faster diagnostics, automated documentation and predictive analytics. Boards are asking about the hospital’s AI strategy. No one wants to be the last organisation in the market still talking about pilots while others claim to have moved into full-scale deployment.

At the same time, many leaders quietly worry that their data is fragmented, their infrastructure fragile and their privacy teams overstretched. That concern is not a sign of being behind. It is, in fact, exactly where the global privacy and cybersecurity community says hospitals should start.

Recent campaigns led by the National Cybersecurity Alliance and guidance from the Office of the Information and Privacy Commissioner in Newfoundland and Labrador have both made the same argument: AI in healthcare is as much a data governance and risk issue as it is a technology question. The message is simple. Before you ask what models to adopt, you should know what data you hold, where it lives, who touches it and how it is protected.

FOMO isn’t a strategy

The fear of missing out on AI is real. Executives see headlines about hospitals experimenting with generative tools, radiology support systems and virtual agents. Yet the same organisations that celebrate AI’s promise also warn that attackers are using similar technologies to craft more convincing phishing attempts, generate deepfakes and exploit misconfigured systems. For hospitals, this does not simply mean more sophisticated spam. It means staff can be tricked into sharing credentials with “tools” that look legitimate. It means third-party AI embedded in workflows can become a new route for data leakage. It means patients may struggle to distinguish real hospital communications from algorithmically generated fakes.

In that context, admitting “our data is a mess” is not an excuse. It is a responsible starting point. Hospitals typically operate multiple electronic records, legacy departmental systems and local data stores that have grown over years without a unified plan. Access rights have been granted piecemeal. Logging, backup and incident response are often inconsistent. Regulators increasingly expect health organisations to address these basics before layering AI on top. A hospital that is consolidating systems, cleaning up shadow databases, tightening vendor contracts and updating privacy impact assessments is not delaying innovation. It is doing the core AI-readiness work.

That is why the most important questions for a board are not “Are we using AI yet?” but rather: what problems do we expect AI to help with, how will we know it worked and could we defend our use of health data in front of a privacy regulator tomorrow? A mature discussion begins with clearly defined goals, such as reducing time to diagnosis in a specific pathway, shortening turnaround for reports, or improving patient communication in one chronic disease. It then asks whether the hospital understands the data flows, has assessed the privacy impact, knows who will be accountable when something goes wrong and has plans in place for breaches and incidents. Only after that should it turn to the details of particular tools and vendors.

The question of third-party risk is central. Every AI tool imported into a hospital brings its own practices, infrastructure and vulnerabilities. Leaders need to understand where data will be processed, whether it will be used to train external models, how consent and legal bases will be handled and how quickly access can be cut off if needed. These are not technical footnotes. They sit at the heart of public trust.

Data Foundations as real AI readiness

Seen from this angle, AI is not a single project. It is the visible tip of a much larger transformation in how hospitals treat data: from something passively accumulated to something actively governed. Cybersecurity, privacy and clinical safety become different faces of the same question: can we rely on our information systems enough to let algorithms influence care?

This is also why Europe has become such an interesting environment for testing AI in hospitals. The combination of strong data protection rules, emerging AI regulation and a diverse landscape of public and private providers creates a demanding, but highly instructive, test bed. An AI solution that can demonstrate value and compliance in a European setting is likely to be robust enough to travel.

Europe as a testbed – if you choose the right partner

Success in this context depends heavily on choosing the right partners: organisations that understand both the clinical reality on the ground and the regulatory framework, and that are able to design pilots which are legally sound, technically feasible and clinically meaningful.

So, is your hospital ready for AI? If readiness means having deployed a chatbot on the website, many institutions will soon be able to answer yes. If it means being able to use AI in ways that are safe, lawful, resilient and genuinely helpful to patients, the bar is higher. The encouraging news is that work on data governance, cybersecurity and privacy-by-design is not a distraction from AI. It is the foundation. Hospitals that invest there now, and that choose experienced partners to guide early AI projects, are not falling behind. They are quietly building the conditions under which artificial intelligence can move from impressive demonstration to trustworthy, everyday practice.
